The backstabber multi is fairly unique, as it checks a fairly wide angle based on the facing direction of the chestbone, and is also a binary damage bonus. He's using a modded version of the knife that has a 1.5x backstabber multi (no weapons in vanilla use this). This is not back damage, it's the backstabber multi.
